The School recognizes its responsibilities under the Republic Act No. 10173, also known as the Data Privacy Act of 2012, with respect to the data collected, recorded, organized, updated or modified, used, shared, retained, retrieved, consulted, consolidated, blocked, erased or destructed from students, employees, alumni, affiliates, contractors, visitors, partners, and vendors.
The personal data obtained from the data subject or an individual is entered and stored within the company authorized information and communication systems for the period allowed under applicable laws and regulations, and will only be accessed by the authorized personnel. COC has instituted appropriate organizational, technical and physical security measures to ensure the protection of the collected personal data.
Furthermore, the information collected shall be shared with and made available to CHED and/or any other interested parties in order to pursue lawful purposes and legitimate interest and comply with legal mandate, and only be used for the purpose of operations and will never be disclosed to anyone without prior consent.

In compliance with the Data Privacy Act of 2012, Cagayan de Oro College (“Company”) issues this Personal Data Protection Notice (“Notice”). Note
- You'll be provided with an account after requesting a document for the first time.
- For submitted Transcript of Records (TOR) from last school attended with remarks/purpose" For Evaluation", "For Reference", or "For Employment", subjects will be temporarily credited and re-evaluated until submission of official TOR with remarks "Granted Transfer Credential" or "For COC Copy".
- Deans or Program Heads of respective colleges shall evaluate and approve the crediting of the following subjects:
- For general Education subjects - College of Arts and Sciences.
- For Information Technology (IT) or computer subjects - College of Information Technology.
- For major/professional/core subjects - concerned colleges.
- Please accomplish this form with attached TOR. Submit original copy to the department and photocopy to Registrar's office

Registrar will send you an email if it is ready to be released or you can view the status of your document in your account.

The system will create an account for you after your first request. No need to worry about that. Just remember the email you have entered when you request a form as that will be used as your email to login to your account.
